Injector 5 is nearly at 0.80 mg, I'm wondering if your CP4 pump is starting to take a dirt nap?
Low rail pressure faults are definitely a Red flag and if it is they your injectors are struggling to atomize fuel properly. Vehicle will run but not very well.

Okay, is there any way of testing that?

I logged some data today (see here) on my Oxygen Sensor and it seems like my Lambda numbers are very high, and sometimes they go up to "30" for periods of time, which seems to be like they are not working correctly. Could it be that my oxygen sensor is faulty and that it could be one of the causes of my issues (blocked DPF filter, riding rough, excess smoke)?
 
A lean idle is normal. Diesels love to run lean.
Your initial complaint was that there was a lot of smoke.... smoke will be present when running RICH.
You need to log lambda under load and rpm, especially whatever conditions you are visually seeing all that smoke under.
 
Check this out..... there's some data from my vehicle as an idea, and a bunch of us are talking about AFRs and stuff in that post if you want to read up on it.

Yes, so that is what I thought originally - if my Lambda is showing that it is running lean tghen why is there smoke, that is counterintuitive. But then I relaized if my Oxygen sensor is faulty and is showing readings of the car being lean (when in fact it would not be) then the EUC would send more fuel into the injector (when it actually does not need it) and therefore create the smoke. In terms of my readings, they seem to be way off. Here is the full sheet, but I will also paste some data below as well. As you can see sometimes it shows the reading as 30, which seems very odd:

Your log is useless as there's insufficient data points.
What shows us there's smoke at any point on there?
What tells us if you're on the throttle or coasting?
What tells us injector rates?
What tells us EGT?
etc, etc, etc........

Okay I agree that I should redo the logs with some of the metrics you have suggested.

My only observation was that the Lambda scores seemed to be way out of what you would expect to see, under any conditions. My understanding is that they are aiming to be around 1ish, but in some cases it is 30. Are you saying that 30 is a value that could be seen under certain conditions?

I am trying to work out why my car produces so much smoke on acceleration. I know that removing the DPF filter would mean that some smoke will come out on acceleration, but this seems a lot.

Here is the full scan - there seem to be a lot of error codes with other modules, many of which speak about low voltage issues.

If you've removed your dpf, your car will smoke like every diesel ever has since the 1800s. the end.

OK, so look at the data.

How old is your battery?
Do you know how to pull the stats on it with VCDS?
So you were right, my battery did need replacing, thanks for the heads up on that.

So after basically giving up on trying to fix the smoke issues for 6 months, I eventually took my car to another service shop (JEG Diesel) to take a look at it. What they picked up was that the new injectors that were put into my car were note encoded, and the guys that replaced my injectors hadn't done a fuel tank clean and they thought that could have caused my issues (there was sediment in my fuel once they removed it). That didn't actually fix my smoke issues so I sent it back, and they then took the car to a Tuning Shop (ATM Tuning) to take a look at the ECU (to see if the mapping had been screwed with by the guys who removed the DPF) and these guys picked up my EGR was faulty (although there was no fault code coming up). Once they fixed that with a plate, I have had ZERO smoke. Makes me wonder if this was the issue the whole time (that blocked the DPF, that caused the Injector Deviations) and that I spent almost USD5000 for nothing.

While my car was at JEG Diesel asked them to also test my old injectors (I thought maybe the previous injectors may have been fine as you had said to me a while back that it looked like I was scammed) and they told me that all 6 of my old injectors had electrical faults and could not be repaired. I found this hard to believe because how did all of them have the same fault, and how was my car even running prior to me replacing them? The cynical part of me thought they might have just been saying this to me so I would leave my injectors with them and they could refurbish them, but maybe I am just paranoid...
 
Refurbished injectors are not good at all. Yes, u have some warranty on it and it is cheaper than new one. But lifespan is way shorter. Here is my experience: I have replaced all four injectors on VW Touran with refurbished. Guess what. Three of them failed after 4 days of driving. I have replaced them again with new. Engine runs smoother, lower fuel consumption and no shaking.
