This is most likely one of the freewheels under the intake manifold, that are used to drive the alternator and the A/C compressor. They're supposed to be replaced (together with the rubber clutch) every 150.000 miles, but I guess your car is well over that mileage.

If it's not the freewheel, then maybe your alternator or compressor that's (ie. the bearing in it) is shot.
 
When you replaced the alternator did you replace the Gates coupling that Gnits referred to?

It's in two parts - there is a metal clutch and a rubber connector.

And there is another Gates coupling on the aircon compressor.

They are there to ensure that if either component seizes it doesn't damage the engine itself.
